different, please pack individual labeled bags of your pet's food for
each mealtime.
Vaccination
Policy:
I understand that in order for my pet to board, he/she must be current
on his/her Rabies, Distemper (and Kennel Cough-dogs only). If you are
not a regular client of Fairfield Pet Hospital, please bring your
records of current vaccinations and medical care from your regular
veterinarian at the time of admission. If my pet is not current I
understand that they will be vaccinated on admission after receiving an
examination by the doctor.
Flea Policy:
I further understand that in order for my pet to board, if fleas are
found they will be treated with one dose of frontline and one oral Capstar
flea treatment upon arrival at the owner’s expense.
Heartworm
Policy:
I further understand that in order for my pet to board, if my dog is not
on monthly heartworm preventative, a negative fecal parasite examination
must be recorded upon arrival and treated if the animal is shedding
intestinal parasite eggs at the owner’s expense..
Medications:It is very
important that we are aware of any medication your pet will need while
staying with us. Please list all medications along with the proper
dosages (this would also include heartworm medication). If you have
forgotten your medication we will gladly refill your prescription. There
is an administration charge for administering medications.
